Digitally enabled neighbourhoods: early learning from best practice
Digitally enabled neighbourhoods have an important role to play in delivering more proactive, integrated and person-centred care. However, successfully implementing neighbourhood models requires the right combination of digital capability, collaboration and organisational change.
In this workshop, speakers from Dorset, Lancashire and South Cumbria, and DHU Healthcare will share early learning and practical examples of how digital tools, data and technology are supporting neighbourhood-based care. Participants will explore key lessons learned, common challenges and opportunities to apply similar approaches within their own organisations and systems.
